24 hour Critical Incident line
This Winnipeg Regional Health Authority patient safety initiative encourages staff, patients and families to report critical patient safety situations - such as a patient who has fallen and may have a fractured hip. At Misericordia, this process allows for a Critical Incident Review Team to review the facts associated with the incident and make recommendations for implementation to prevent the same event from reoccurring.

It’s Safe to Ask
In May 2007, Misericordia launched The Manitoba Patient Safety Institute initiative, It’s Safe to Ask. Patients and families are encouraged to enhance their safety and quality of life by asking a health care professional these three questions:
- What is my health problem ?
- What do I need to do ?
- Why do I need to do this ?
Medication Reconciliation
Misericordia has joined the Safer Healthcare Now! campaign by collaborating with the Winnipeg Regional Health Authority patient safety initiative, Medication Reconciliation. This national quality improvement initiative looks at enhancing patient safety in healthcare settings. It provides an up to date accurate medication list, which is essential to ensure safe medication prescribing in any setting. At Misericordia, both our Acute Care and Long-Term Care programs are actively involved in meeting this patient safety need.
Executive Walkrounds
The aim of patient safety is to move from a culture of blame, fear and retribution to one of a safety culture that encourages open discussion surrounding incidents, asking questions and making improvements. Executive walkrounds provides a structured dialogue between executives and front line providers that promotes a mutual understanding of patient safety issues facing them in their daily work. Misericordia is embarking on this initiative, in collaboration with Workplace Safety and Health, with the goal to bring all safety issues to the forefront.
