Personal Health Information Privacy
Personal health information is collected for the provision of health care or payment for health care.
Personal health information includes:
- your name, age, address, and Personal Health Identification Number (PHIN)
- details about your health, health care history and the health care you have received
- details about payment for the health care you have received.
We will use your personal health information that we have collected in accordance with The Personal Health Information Act and share it with only those who need to know the information. Examples of use by the MHC may include:
- providing, monitoring or evaluating your health care while you are at MHC
- teaching and training health care students
- research and planning that relates to the provision of health care by MHC
We will only disclose your personal health information to others with your consent unless otherwise authorized by The Personal Health Information Act. Your rights related to your personal health information include:
- examining your personal health information - receiving a copy of your personal health information
- requesting your personal health information be corrected
- giving permission for your personal health information to be shared with others when this is required by the Personal Health Information Act
- making a complaint to the Provincial Ombudsman's Office about access to or about collection, storage, use or disclosure of your personal health information.
